PRIVACY COLLECTION INFORMATION NOTICE FOR PATIENTS

Your clinician collects your personal information to provide clinical services to you. As part of their services, they may use a software within this Coviu platform called Coviu Assist to record consultations and prepare file notes of your consultation so that they can provide you with the best care. How Coviu Assist Works

Coviu Assist is a cloud-based software provided by Coviu Global Pty Ltd (ACN 608 925 060). You can let your clinician know that you are comfortable with them using the Coviu Assist software verbally or by clicking ‘accept’.

You have the right to decline the use of Coviu Assist software for your consultation. Your decision will not affect the quality of care you receive from the clinician.

If you are accompanying a minor or a person who cannot give legal consent, you can still agree for the health professional to use the Coviu Assist software provided that you are their parent, legal guardian or are otherwise authorised (Authorised Representative).
