Online patient forms FAQs
Online patient forms are the paperwork a practice collects from a patient digitally instead of on paper: registration and demographic details, medical history, consent, referral information, scored assessments, and financial agreements such as a Medicare assignment of benefit. Rather than printing a pack or emailing a PDF, the practice sends a link the patient completes on their phone or computer before they arrive.
No, not unless you want to. Send us the form you already use – a PDF, a Word document or a scan – and we'll set it up as a Coviu form. Or you can build your own. Most practices do some of each, and in either case, you’ll be able to change it whenever your paperwork changes.
Gentu, MediRecords, Halaxy, Nookal and Cliniko are supported today, with completed answers written back into the patient record. How much writes back varies a little by system, and we'll show you exactly what yours does. We're adding more systems based on what practices in the program are running, so tell us yours when you sign up.
No. Patients receive an SMS with a secure link, complete the form in their phone browser and submit it. There is no app to install, no account to create and no password to remember, which is what makes it work for older and less confident patients.
You can set the reminders per appointment type – say one after a week and another, two days before the appointment. Anything still outstanding close to the appointment is flagged, so you see it in advance rather than on the day of.
